内容摘要：This paper examines how a nationwide price control reform in China-the "4+7" centralized drug pro-curement pilot-affected physician behavior in a system where provider income is partially tied to drug mark-ups. Leveraging administrative data from a tertiary public hospital (2018-2020) and exploiting heterogeneity in departmental exposure to procurement-targeted drugs, we implement a difference-in-differences strategy to estimate causal effects. While the policy reduced targeted drug prices by over 35%, total prescription spending remained unchanged. Instead, we observe a sharp substitution toward unregulated revenue channels: physicians in high-exposure departments increased their use of traditional Chinese medicine (TCM) by 1.6-1.9 percentage points and extended inpatient stays by over 7%. These responses are especially pronounced among senior physicians and for out-of-pocket patients. We further document a modest but statistically significant decline in patient recovery rates, suggesting that revenue-preserving strategies may compromise care quality. Our findings demonstrate that narrowly targeted procurement reforms can trigger offsetting behaviors when provider payment systems are misaligned, underscoring the need for incentive-compatible design in health system reform.
